What They're Looking For.
Must Have
Minimum one (1) year experience, Updated Tuberculin (TB) test, Current CPR certification, Clear Criminal History/Drug Screen, Critical thinking, Task competence, Ability to think on your feet, Problem solving skills, Ability to multi-task
What You'll Do.
Admit clients to services
Ensure standards of care
Provide training and support
Work with Executive Director
Perform light office duties
Build relationships with staff
Build relationships with residents
Build relationships with families
Build relationships with Community Directors
Perform assessments of new residents
Perform assessments of discharged residents
Maintain contact with clients
Maintain contact with therapists
Maintain contact with Regional Care Manager
Maintain contact with family
Maintain contact with healthcare team
Assure effective care coordination
Make appropriate resource referrals
Prepare medical documentation
Prepare case conferences
Prepare client contacts
Prepare medication order changes
Prepare re-certifications
Prepare progress updates
Prepare care plan changes
Prepare visit/shift reports
Update client records
Summarize client records
Confer with other health care disciplines
Develop initial plan of treatment
Provide hands on care
Work the floor as support
Process weekly payroll
Create weekly scheduling
