Eligibility Requirements

physical ability to perform job related duties which may require lifting, standing, bending, stooping, stretching, walking, pushing and pulling with or without reasonable accommodation, On-Call after hours/weekends, Clear Criminal History/Drug Screen, Updated Tuberculin (TB) test

What They're Looking For.

Must Have

Minimum one (1) year experience, Updated Tuberculin (TB) test, current CPR certification, Clear Criminal History/Drug Screen, Critical thinking, task competence, the ability to think on your feet, Problem solving skills, the ability to multi-task

What You'll Do.

assesses the needs of the client in the community setting and admits to services

responsible for ensuring our standards of care are provided by the HHA/ CNA

working side by side with your clients getting to know them on a personal level

responsible for the entirety of their community

Ensuring staff has all the support needed to provide our high standards of care by providing training and on going support

working along with the Executive Director

Hiring/Training/Supervising Your Staff

Building Relationships with Staff

Families and Community Directors

Assessments of new residents or those being discharged back from a hospital/rehab center

Acts as a liaison and maintains contact with clients

Regional Care Manager

family and other members of the healthcare team and the community in assuring effective care coordination and appropriate resource referrals

Prepares appropriate medical documentation on all clients

including any case conferences

medication order changes

progress updates and care plan changes

Prepares visit/shift reports

updates/summarizes client records and confers with other health care disciplines in providing optimum client care

Confers with the client

and family in developing the initial plan of treatment based on client needs and initial patient assessment

Provides hands on care

management and evaluation of the care plan and teaching of the client under the Regional Care Managers supervision

Coordinates appropriate care encompassing various healthcare personnel (such as Physical Therapists

Occupational Therapists

Home Health Aides and external providers)

Weekly Payroll and Scheduling

Team & Collaboration

working side by side with your clients getting to know them on a personal level; working along with the Executive Director; Building Relationships with Staff, Residents, Families and Community Directors; Acts as a liaison and maintains contact with clients, therapist (s), Regional Care Manager, family and other members of the healthcare team and the community in assuring effective care coordination and appropriate resource referrals; confers with other health care disciplines in providing optimum client care; Confers with the client, physician, and family in developing the initial plan of treatment; working and learning with an interdisciplinary team of care providers

Communication Scope

Building Relationships with Staff, Residents, Families and Community Directors; Acts as a liaison and maintains contact with clients, therapist (s), Regional Care Manager, family and other members of the healthcare team and the community in assuring effective care coordination and appropriate resource referrals; Confers with the client, physician, and family in developing the initial plan of treatment
