Eligibility Requirements

Provide on-call crisis services on a rotating schedule (including both primary on-call responsibilities and back-up on-call responsibilities)—as directed by the Team Leader—to cover program needs., Work weekend, evening, and holidays on a rotating schedule—as directed by the Team Leader—to cover program needs., Heavy work exertional activity., Occasionally lift and/or carry up to 50 lbs., Sit, stand, and/or walk at least 6 hours in an 8-hour workday., Stooping (bending at the waist) and crouching (stooping and bending at the knees)- required handling (grasping)- required fine manipulation- required occasionally., Good physical and mental health., Visual and auditory acuity required to provide comprehensive care.

What They're Looking For.

Must Have

Minimum of two years of direct-service experience with individuals with mental health concerns, English fluency required in speech, reading, and writing, Clean driving record and active driver’s license required, Bachelor’s degree with major coursework in criminal justice, sociology, social work, psychology, counseling, or related field

Nice to Have

Degree in rehabilitation counseling preferred, Experience working with individuals with forensic involvement and/or co-occurring substance use disorders is preferred, Additional fluency in a second language (e.g., Spanish) preferred

What You'll Do.

Provide supportive services—including outreach

assessment and service planning

support network building

consultation to families

side-by-side skills training and instruction in the use of community resources

including referrals to community assistance

Assist ACT participants with obtaining and maintaining the skills needed to attend school

and/or work in the community.

Assist with activities of daily living

Provide medication service—including delivery of medications and observation of ingestion—as directed by the team psychiatrist

Provide individual and supportive counseling to assist with symptom management.

Complete documentation of all services in a timely manner

using the electronic health record

in accordance with program and Network standards.

Cross-train with other ACT Specialties to provide other services (e.g.

etc.) to the ACT participants.

Develop relationships with various employers within the community and assist with matching participants with jobs in their chosen area of work.

Offer on-the-job support and coaching—in line with the philosophy of the Supported Employment model.

Assess the participants’ educational and work history

as well as the participants’ goals for future vocational engagement.

Develop a plan to assist ACT participants with reaching their vocational goals.

Provide educational and support groups to participants of the ACT Team—to address vocational and educational goals.

Provide primary support to ACT participants and provide cross-training to the other members of the ACT team—to assist with access to housing-related supports and resources—with the goal of promoting mental health stability and reducing forensic and hospital recidivism.

How You'll Work.

Team & Collaboration

Member of a multidisciplinary Assertive Community Treatment Team; In collaboration with other members of the ACT staff, provide service coordination for an assigned group of clients.; Cross-train with other ACT Specialties to provide other services (e.g., housing support, vocational support, etc.) to the ACT participants.; Provide cross-training to the other members of the ACT team—to assist with access to housing-related supports and resources

Communication Scope

English fluency required in speech, reading, and writing

Process & Methodology

Assume responsibility for developing, writing, implementing, evaluating, and revising treatment goals.
