What You'll Do.
Serve as initial point of contact

Coordinate patient admissions
Coordinate patient transitions across levels of care
Provide administrative support to treatment teams
Ensure timely distribution of assessments
Ensure timely completion of assessments

Send welcome emails or information to patients/families
Identify referrals for varying levels of care

What you’ll be doing: Schedule: Full Time; Monday - Friday 8:30 AM - 5:00 PM The Treatment Coordinator serves as the initial point of contact for patients, families, and the public, ensuring they are greeted and assisted in a professional and supportive manner. This role plays a critical part in supporting a positive patient experience by coordinating patient admissions and transitions across levels of care.
