Eligibility Requirements

Valid driver's license, state-required auto insurance, Required to travel to sites extensively (> 50% of the time), Ability to handle confidential information, Maintains patient privacy, Sensitivity to service population’s cultural and socioeconomic characteristics, Influenza immunization or declination, COVID-19, MMR and Tdap immunizations, Education verification, Reference check, Background fingerprint clearance (FBI, DOJ, CACI), Drug and alcohol screening, Tuberculosis screening

What They're Looking For.

Must Have

Master’s Degree in Social Work, Psychology, Marriage and Family Counseling, or a closely related field, Master’s Degree in a human services field, Bachelor’s degree and 4 years’ experience providing case management services, Master’s degree and 1 year of experience providing case management services, Valid driver's license and state-required auto insurance, Required to travel to sites extensively (> 50% of the time), Ability to handle confidential information, Maintains patient privacy, including protecting the confidentiality, safeguarding the integrity and availability, and limiting the use and disclosure of patient Protected Health Information (PHI) to authorized individuals and entities, Participates in, conducts, and sustains performance quality improvement activities according to designated role and responsibilities, Sensitivity to service population’s cultural and socioeconomic characteristics, Working knowledge of computer programs in a Windows environment, including Word, Excel and E-mail, New Hire Orientation, Mandated Reporter, AB-1343: Sexual Harassment Prevention training

Nice to Have

Case Management experience preferred, Early Childhood Development experience preferred, Experience working with children, youth, and families involved in the child welfare system preferred, Case management experience preferred, Experience working with children, youth, and families involved in the child welfare system preferred, Experience working with children, youth, and families involved in the child welfare system preferred, Spanish / English bilingual preferred, WRAP program only – Wraparound basic training, Family Preservation program only – Family Preservation basic training

What You'll Do.

Provide direct services

Conduct regular home visits

Provide skills building

Assist in crisis intervention

Provide case planning

Ensure timely and quality documentation

Represent CII at DCFS

Establish and maintain rapport

Assess and identify participant’s needs

Develop meaningful goals

Measure program outcomes

Provide community referrals

Tailor services and supports

Advocate on behalf of the participant

Participate in integrated case conferences

Monitor individual needs and services plans

Ensure effective quality clinical service delivery

Complete outcome and evaluation testing

Develop and maintain effective working relations

Conduct intake interviews

Complete required paperwork

Increase professional knowledge

Establish personal benchmarking

Participate in in-service programs

Maintain knowledge of service standards

Maintain knowledge of policies
