Full Job Description
At Rebecca School, we support students ages 3–21 with neurodevelopmental delays through the DIRFloortime® model. Guided by values of respect, relationships, and emotional growth, we help students connect and thrive. Join us and make a lasting difference in their lives. As a Transition Coordinator, you will play a vital role in helping students prepare for life beyond school by supporting transition planning, independence, community engagement, and long-term life goals. You will work closely with students, families, and multidisciplinary teams to develop individualized plans that promote growth, self-advocacy, and successful post-school outcomes. Through collaboration, resource coordination, and program development, you will help students build the skills and connections needed to thrive in their communities and future endeavors. What You’ll Need - Bachelor's degree in Education, Special Education, Social Work, Psychology, Human Services, or a related field preferred - Experience working with adolescents and young adults with diverse learning, developmental, or behavioral needs - Strong communication, collaboration, and relationship-building skills - Knowledge of transition planning, community resources, and person-centered planning practices - Ability to coordinate multiple projects and manage competing priorities - Experience working with families, school teams, and community partners - Strong organizational and documentation skills - Proficiency with Microsoft Office and Google Workspace What You’ll Do - Maintain a transition planning caseload of students ages 14–22 and collaborate closely with students, families, and educational teams - Lead the development of long-term life planning goals and short-term transition objectives - Coordinate the School Independence Program (SIP) and support student participation - Partner with educational supervisors on curriculum planning, community experiences, and the Best Buddies program - Assist with planning and implementing e
