What They're Looking For.
Must Have
Certified and licensed Nurse Practitioner in ANCC (or equivalent) in Adult/Family/Geriatric/Acute, Eligible for Medicare company bills on behalf of, 2+ years NP experience with complex adult populations in home health or post-acute, able to practice independently, Comfortable conducting telehealth visits and navigating virtual care platforms, English & Spanish required
Nice to Have
Google Suite, multiple EMRs (HCHB experience a plus), Slack, AI transcription/tools, Spanish preferred
What You'll Do.
Conduct remote assessments for home health patients
act on predictive health signals in real time
coach care teams through virtual touchpoints to drive earlier interventions and reduce preventable hospitalizations
Conduct initial and follow-up telehealth assessments on flagged patients
initiate/adjust plans of care
coordinate with referring and primary care providers—all via secure video/audio platform
Interpret trend-based insights from the Circadia platform
prioritize high-risk patients
and drive timely interventions to prevent decline and unnecessary ED visits
Complete documentation/charts within the same day
Efficiently complete 25+ tele visits per day for designated home health patients in the SoCal/OC area
focusing on new admissions
high-acuity flagged patients
Surface insights to improve virtual workflows and tech integration in a home health setting
Support NP onboarding
and partner with clinical ops/product teams
