We are backed by leading healthcare and AI investors and headquartered in El Segundo, CA.   ## The Role Cover weekend tele visits for home health patients in the SoCal/OC region. As a per diem After Hours tele NP, you'll conduct remote assessments daily 5PM to 11PM, act on Circadia platform alerts, and ensure continuity of care for high-risk patients between weekday care team touchpoints. ## What You’ll Do After Hours remote patient care: Conduct follow-up and urgent telehealth assessments on flagged patients; adjust plans of care as needed; coordinate with on-call providers and care teams. Data into action: Interpret trend-based insights from the Circadia platform, prioritize high-risk patients, and intervene on weekend alerts to prevent unnecessary ED visits or hospitalizations. Close the loop: Complete documentation/charts same day via AI-powered tools. After Hours tele-visit management: Efficiently complete tele visits for designated home health patients in the SoCal/OC area on after hours, focusing on Circadia alerts and high-acuity flagged patients. Quality & product feedback: Surface insights to improve virtual workflows and tech integration in a home health setting. ## How Success is Measured (KPIs) Visits: Target 25+ after hours tele visits/day Flagged patients: Time to Acknowledgement Rate less than 24 hour
