What You'll Do.
Connect and engage with people
Start relationship with interested individuals
Educate patients and caregivers
Assist with application and enrollment
Complete eligibility process
Onboard patients and caregivers
Guide patients and caregivers through onboarding
Conduct virtual or in person orientations
Guide patients and caregivers through home care journey
Ensure patients receive needed care
Ensure patients feel heard and cared for
