Must Have
Active Texas SLP license (in good standing), clinical reasoning and ability to operate independently in the field, Comfort identifying red flags and escalating changes in condition to RN/MD, caregiver education skills (teach-back, carryover planning), communication and documentation, Reliable follow-through on orders, frequencies, and patient goals, Current driver’s license and comfort with daily travel across assigned territory
Nice to Have
Prior home health experience, Homecare Homebase (HCHB) experience (or similar home health EMR), High-signal neuro experience (stroke, aphasia, cognitive impairment), Dysphagia-related experience appropriate for home health
What You'll Do.
Complete comprehensive in-home SLP evaluations
deliver skilled SLP visits
Assess and treat (as ordered/clinically indicated): Aphasia and other communication disorders
Cognitive-communication impairment (attention
executive function) impacting safety and independence
Dysphagia screening/assessment and swallow safety education
Build and execute a clear plan of care with measurable
Coach patients/caregivers with practical strategies and routines
Document promptly and thoroughly in Homecare Homebase (HCHB)
How You'll Work.
Team & Collaboration
Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.
