CCC-SLP - American Speech-Language-Hearing Association (ASHA) or presently active in a supervised clinical fellowship year and certification received within one year of employment, Speech-Language Pathologist - Licensing Board An active license in the state of practice

1-3 years Speech Language Pathologist in a hospital, SNF, outpatient and/or home health setting

Work where you matter.** **A Brief Overview** Evaluates patients, develops treatment plans and provides treatment for medically necessary speech therapy services addressing a patient's deficits in speech, language, cognition, voice and dysphagia in the home setting. The Speech Pathologists demonstrates the knowledge and skills necessary to provide care for the population served. **What you will do ** * Evaluates assigned patients through selection, administration, and interpretation of formal and informal diagnostic assessments to determine the levels of functioning and intervention needs in all areas of speech pathology. * Demonstrates knowledge of age-specific factors and performs satisfactorily related to patient’s age infant through geriatrics as noted on unit specific guidelines and competencies. * Develops and implements individualized treatment programs for the patients needing services. * Establishes and modifies intervention goals and strategies for patients/ treatment program based on on-going evaluations. Uses selected activities and/or tasks to promote and maintain health, and to achieve treatment goals. * Provides direct therapy services to patients using various functional settings and strategies. * Plans therapy with in the patient’s individual physical, emotional and intellectual capabilities as indicated and coordinated with the rehabilitation, medical, and nursing care plans. * Prepares written documentation as required by the profession and the department, such as evaluation results, individualized treatment plans, progress notes, and other s
