**Department:** 37650 Navicent Health Medical Center - Inpatient Rehabilitation **Status:** Part time **Benefits Eligible:** No **Hou****rs Per Week:** 0 **Schedule Details/Additional Information:** Days and weekends **Pay Range** $38.20 - $57.30 **Major Responsibilities:** Patient Assessments / Reassessments  Performs appropriate patient assessments resulting in an effective treatment plan.  Reassesses patients at appropriate intervals of time demonstrating the ability to observe treatment effect and modify the plan of care as indicated.  Involves the patient and /or family in the assessment process and the formation of the care plan. Care Planning / Interdisciplinary Communication  Provides care in accordance with physician’s orders.  Demonstrates appropriate case management to ensure quality of care and best patient outcome.  Communicates appropriately to physicians, nurses, case managers, and other members of the health care team to ensure care is collaborative and appropriate. Patient Outcomes  Assesses and documents patient outcomes criteria at the initial and discharge evaluations. (all shoulder, back, incontinence, and anodyne). Speech Therapy Modalities / Patient Treatment  Demonstrates knowledge of speech therapy modalities / treatments and performs correctly. Operates Equipment Safely and Correctly  Demonstrates knowledge of and competency in using equipment pertinent to providing speech therapy services. Patient Education  Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning.  Includes the family in teaching as appropriate. Documentation  Documentation meets current standards and policies.  Documentation is legible.  No inappropriate abbreviations are used.  Documentation is completed in a timely manner.  Medical Records are closed as per policy. Charge Capture  Time is adequately documented in the assessments and daily treatment notes.  Each charge bill adequately reflects all services
