Must Have

Associates degree required. New Jersey State Nursing License and BLS certification required. 0 – 1 years experience. Ability to communicate in English, both orally and in writing. Ability to perform diverse work assignments with time limitations with a high degree of accuracy.

Nice to Have

Bachelor’s degree preferred. 2-4 years experience preferred. Experience with Standard Office Equipment (Phone, Fax, Copy Machine, Scanner, Email/Voice Mail) preferred. Experience with Standard Office Technology in a Window based environment preferred. Experience with patient care equipment preferred. Experience with EHR, Practice Management System and Departmental/Ancillary Systems preferred.

What You'll Do.

Provide individualized team-based care under the direction of the Primary Care Provider, Clinical Practice Manager and Office Supervisor.

Facilitate coordination of individualized total patient care through the nursing process.

Provide leadership and clinical expertise in assessing, screening, planning, evaluating, educating, and delivering care.

Assist physician and staff with daily coordination of patient care and physician access.

Function as a liaison between patients, staff, and physicians.

Maintain efficient patient flow.

Deliver direct patient care within the scope of licensure and in accordance with SMG policy.

Collect lab specimens, perform Point of Care testing, EKGs, and phlebotomy.

Prioritize patient care by implementing physician orders and nursing procedures.

Ensure proper preparation for patient visits.

Manage Nurse Visit schedule.

Perform patient intake, check-in/check-out.

Communicate problems, concerns, or issues to the Office Supervisor/Manager.

Assure correct patient identification.

Utilize Tasking in EHR.

Manage Clinical Nurse/Assigned inbox.

Obtain vital signs.

Administer injections.

Perform patient outreach for quality measures.

Coordinate and facilitate daily huddles.

Demonstrate telephone triage protocols.

Schedule appointments.

Monitor daily hospital discharge list and collaborate with Care Management.

Adhere to universal safety precautions.

Review and update medication list in EHR.

Initiate and follow through with prior authorization requests, DME orders, home health referrals.

Enter orders and schedule appointments in PMS and EHR.

Provide referrals and/or consult orders.

Check and maintain logs for emergency equipment.

Facilitate transition of care to UCC/Hospital.

Maintain proficiency with department specific equipment.

Actively participate in pilot programs.
