The program brings together health and psychosocial expertise to address the full range of factors that influence wellbeing, with a strong focus on equity, access, and person-centred care. Your role, your impact As a Social Worker in the My Care Partners Program, you will support people navigating the psychosocial challenges that often sit alongside chronic and complex health conditions. Your work will help reduce barriers to care, strengthen individual resilience, and support people to feel safer, more connected, and more empowered in their daily lives. Working closely with Registered Nurse Care Coordinators and primary care teams, you will contribute to integrated, holistic care that responds to both health and social needs.
