What They're Looking For.
Must Have
Bachelor’s degree in Social Work, providing eligibility for AASW membership, Minimum two years’ experience working as a qualified Social Worker, Current driver’s licence and willingness to travel for community and home-based work
Nice to Have
Experience working in community or primary health settings, Knowledge of the social model of health and social determinants of health, Experience working with culturally and linguistically diverse communities, Confidence supporting people to navigate complex service systems
What You'll Do.
Deliver psychosocial support and care coordination
Conduct holistic psychosocial and risk assessments
Develop and implement individualised care plans
Provide short to medium-term therapeutic support
Support goal setting and self-management
Assist with care navigation
Coordinate access to supplementary services funding
Participate in case conferencing
Maintain accurate clinical documentation
Contribute to quality improvement initiatives
How You'll Work.
Team & Collaboration
Working closely with Registered Nurse Care Coordinators; Working with primary care teams; Working within multidisciplinary health teams; Working in primary care settings; Participate in multidisciplinary meetings
