What They're Looking For.

Must Have

Master's Degree in Social Work, LSWAIC, LMHC, LMFT, LASW, LICSW, AAC as required by department, Experience working in a hospital/acute situation or equivalent social service agency, experience relative to the medical social work function

Nice to Have

BLS Certification may be required

What You'll Do.

aiding participants and their families with personal and environmental difficulties that predispose illness or interfere with remaining in their home environment

administer assessment tools and gather data for assessment

assess for optional needed services

interview families to determine social work needs of the participant

contact community resources to coordinate services

provide initial and on-going assessment of participant's social

emotional and cognitive status and needs

ensure accurate documentation is maintained for all assessments
