What You'll Do.
Assist patients and families with complex psychosocial needs
Conduct psychosocial assessments
Execute a plan for patients requiring alternate site of care post discharge
Communicate with alternate sites of care and/or community agencies
Complete and document assessments and plans in the electronic medical record
Provide consultation and resources to the healthcare team
Maintain knowledge of community resources
Participate in department
or statewide stakeholder committees
Develop and maintain knowledge of federal and state regulations
Participate in safety initiatives and risk mitigating measures
Complete safety related competencies and requirements

Work collaboratively with members of an interdisciplinary team; Provides consultation and resources to members of the healthcare team
