Bachelor of Social Work, BLS Basic Life Support, Licensed as a Social Worker, 1 year of experience in clinical setting
Nice to Have
Master’s degree in social work or healthcare related field, Accredited Case Manager Certification, Certified Case Manager, 3 year of experience in an acute care clinical setting, Ambulatory or post-acute, care coordination experience

Provide interventions and discharge planning
Facilitate patient transition

Provide clinical supervision
Identify and prioritize patients
Monitor and revise plan
Complete all necessary documentation
and timely documentation
Incorporate patient's and caregiver’s treatment goals
Follow standardized practices
Support denial prevention
Remove barriers to progression of care
Participate in Interdisciplinary Discharge Rounds
Assist in removing barriers
Ensure resources are utilized appropriately
Offer alternatives to acute care
