What They're Looking For.
Must Have
Masters Degree in Social Work, Registration with the Ontario College of Social Workers and Social Service Workers, Experience in ambulatory care, Experience working with chronic kidney disease populations
Nice to Have
Membership in the Ontario Association of Social Workers, Three (3) years experience working with an inter-professional team in a hospital setting, Experience working with individuals with complex chronic health conditions, Experience working with geriatric populations, Research experience/education, Student education/supervision, Knowledge of a second language
What You'll Do.
Assessing patients for need
Implementing intervention plan with patients
Reassessing and modifying intervention plan
Liaising with community agencies
Completing applications for ALC facilities
Rendering services for hospital benefit
Providing clinical teaching
Participating in research activities

Collaboration with interdisciplinary team; Liaising with community agencies; Contact with inter-professional team; Exchange information relating to patients
