Must Have
Masters in Social Work, State required license, Meets scope of practice board and licensure requirements
What You'll Do.
Assess patients' psychosocial status
Participate in care planning
Utilize FKC patient education programs
Provide monitoring and interventions
Provide supportive counseling
Provide educational counseling
Provide information and assist with referral
Maintain current knowledge
Assess patient awareness of advance directives
Participate in discussion of patient DNR status
Adhere to FMCNA Social Work Policy
Document based on MSW interaction
Provide psychosocial support
Participate in Quality review meetings
Report on quality indicators
Assess patient knowledge of kidney disease
Provide education tailored to patient needs
Provide information about treatment modalities
Facilitate transplant referral process
Provide education to patient/family
Review patient rights and responsibilities
Address immediate needs/concerns
Understand referral and admission process
Support clinic with patient scheduling
Interview patient for discharge concerns
Share causes/concerns with leadership
Liaise with Insurance Coordinators
Provide information about payment to dialysis
Collaborate on changes to patient state
Refer patients to PBS department
Assist with interview process
Maintain updated patient resource lists
Provide training to staff
Contribute to weekly team huddles
Discuss urgent patient issues
Adhere to work defined caseload guidelines
Perform other related duties
