What You'll Do.
Carry out community-based assessments
Identify opportunities to maximise independence
Reduce care package requirements
Manage a varied caseload
Work closely with service users
Work closely with carers
Work closely with multidisciplinary professionals
Complete Occupational Therapy assessments
Identify and recommend equipment
Recommend adaptations
Recommend interventions
Review functional abilities
Assess ongoing support needs
Support efforts to reduce waiting lists
Contribute to positive outcomes
Maintain accurate records
Maintain case documentation
