What You'll Do.
Guide the implementation of the RHPP program
and provide day‑to‑day support to a team of RHPP
Serve as a subject matter expert in RHPP service delivery
Provide daily oversight of active caseloads
and compliant casefile documentation
Assist with preparation for audits
monitoring activities
and all in‑person monitoring visits.
Lead weekly RHPP team meetings
Provide individualized 1‑on‑1 support
and corrective guidance to RHPP staff
and program guidance from management to the RHPP team.
Attend community and stakeholder meetings
Provide field instruction
and practical guidance to assigned Social Work interns
Conduct comprehensive client assessments to identify needs
and collaborate with clients to establish individualized goals.

Provide high‑quality case management services to assist clients in meeting program benchmarks and achieving individualized goals.
Support clients in navigating complex social service systems
