Reassesses and documents the effectiveness of intervention as per guidelines and evaluates diagnosis/ problems on the plan of care based on the patient’s status.
Communicates plan of care clearly in an effective and timely manner to patient
family and the appropriate team members.
Begins discharge planning upon uses available resources to assist in preparing for discharge. Involves patient and patient support network in plan of care.
Manages own work and time and prioritizes assignments to ensure work assignments are completed on time.
Adheres to University and unit-level policies and procedures and safeguards University assets.
