What They're Looking For.
Must Have
Graduate from an accredited school of Professional Nursing, Current state licensure as a Registered Nurse (RN), One year of current clinic experience as an RN, One year of experience in an acute med/surg or critical care setting, Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross, Knowledge of JCAHO and Medicare standards and regulations, Ability to travel to hospitals and community referral sources

Bachelor's of Science in Nursing (BSN)
What You'll Do.
Process inpatient referrals into hospice
Ensure continuity of care
Coordinate discharge planning in contract facilities
Educate potential customers about hospice benefits
Ensure patients understand hospice benefit
Process inquiries for all new referrals
Order medical supplies and DME
Hand off communication with the field team
Document in the electronic medical record
Perform assessment of potential patients
Perform admission assessments for new admissions
Document information in the EMR
Plan the delivery of patient care
Provide comfort and symptom management
Perform daily assessment
Coordinate the plan of care with inpatient staff
Assess patient for safety risks
Assess and document pain
Manage patient’s pain
Document changes in patient condition
Document effectiveness of care
Document timeliness of interventions
Evaluate plan of care
Facilitate achievement of expected outcomes
Know medication action
Know medication dosage
Know medication indications
