Current RN licensure in the state of SD or “multi-state privilege” to practice in SD if residing in a compact state outside of SD, Current CPR certification or completion upon next available program

What You'll Do.
Coordinates patient care and staffing on a daily basis
Assisting with care planning
nursing staff and patient scheduling
supply/equipment ordering and management
nursing daily direction and leadership
Provide care/service in accordance with regulatory and accrediting agencies’ standards
Performing technical procedures necessary for the job responsibilities of a Nurse
Acts as a patient/family advocate and intervenes if necessary to ensure that patient care is not compromised
records significant conditions and reactions and notifies supervisor or physician appropriately
Monitors follow-up and provides further intervention as needed
or delegates patient/family and staff teaching at a level of expertise and documents appropriately
noting the recipients’ participation and response
Identifies problems/concerns and assists in developing resolutions in a positive
Completes all mandatory competencies identified for the job and department
Completes department specific orientation and annual unit skills inventory checklists within appropriate time frames
Completes computer competency checklist and remains current with computer updates
Responsible for coordination and oversight of Chronic Care Management (CCM) services for patients with multiple chronic conditions
Assists in vaccine coordination
Organize staff for scheduling and meeting purposes
Assess skills and capabilities of staff and assist in education and competency training to address needs identified

This individual is responsible for assisting with care planning, procedures, staff education, nursing staff and patient scheduling, supply/equipment ordering and management, nursing daily direction and leadership, work with and report directly to Director of Patient Services.
