**Posting Date** 06/03/2026 3401 South 19th St, Tacoma, Washington, 98405, United States of America **This DaVita IKC Nurse position is for the Washington region and is a remote-based position.** **_Job Description:_** **IKC RN Case Manager** * **Arrival** : Typically starts from home, checking emails/dashboard for hospitalized or high-risk patients. * **Primary Tasks** : * **Caseload Management:** Reviewing a roster of high-risk patients (often ~100+). * **Field Visits:** Traveling to dialysis centers or nephrology offices to meet patients _during_ their treatment/appointment. * **Assessments:** Conducting Comprehensive Health Evaluations (CHEs)—head-to-toe assessments covering medical, social, and emotional needs. * **Education & Coordination:** Teaching patients about comorbidities (diabetes, heart failure), reconciling medications, and arranging follow-up care to prevent hospital readmissions. * **Documentation:** Heavy focus on documentation for risk adjustment and care plans (often completed from home). * **Outcome Focus:** Reducing hospitalizations and managing overall health _long-term_. * **Care coordination along with Nurse Practitioner and Care Coordinator.** DaVita Integrated Kidney Care (DaVita IKC) is an the integrated care division of DaVita Inc. working on DaVita’s vision to provide integrated care to all ESRD patients, who are some of the most medically complex and vulnerable patient populations in the US.
