Develop goals for high-risk patients
Plan interventions for high-risk patients
Maximize patient outcomes
Provide care coordination
Provide disease management coordination
Identify patients with coordination needs
Conduct comprehensive clinical assessments
Inform patients about care coordination
Work with patients on care coordination
Provide education to patients
Provide coaching to patients
Monitor patient compliance with care plan
Perform reassessments regarding patient progress
Serve as liaison for patients
Serve as advocate for patients
Assist in managing transitions of care
Ensure optimal communication during transitions
Ensure optimal planning during transitions
Identify barriers to care
