We are so glad you are interested in joining Sutter Health! **Organization:** SCAH-Sutter Care at Home - Bay **Position Overview:** This role will bridge the gap between SCAH Home Health and SCAH Hospice partners across 9 locations. Ideal candidate will have experience in one or both clinical areas with preference given to RNs with project experience. Provides leadership in managing and directing the program and is responsible for implementing and growing program internally and externally. Is responsible for operational roll out and day to day oversight of the program. Coordinates outreach, oversees data management, conducts data analyses and reporting out of program results, leading improvement activities with all stakeholders. Acts as liaison and manages relationships, ensuring satisfactory execution of all components of the program.
