As part of the Care Dimensions team, you’ll gain the support and inspiration for a career you’ll find meaningful every day. 36 hours or full time Days and hours are flexible Reporting to the Director of Admissions or Clinical Manager – Admissions, the RN Admission Liaison Nurse plays a critical role in ensuring a smooth and compassionate transition into hospice care. This position is responsible for conducting patient assessments and facilitating admissions while ensuring continuity of care with the interdisciplinary team (IDT). You will: Conduct comprehensive patient assessments to determine eligibility and appropriateness for hospice care Coordinate and facilitate seamless admissions into hospice services Provide education to healthcare professionals, patients, and families on hospice philosophy, services, and benefits Build and maintain strong relationships with referral sources to support growth and access to care Actively identify and develop new referral opportunities while strengthening existing partnerships Collaborate closely with internal clinical teams to enhance service delivery and patient outcomes Serve as a key liaison between Care Dimensions and hospitals, facilities, and community providers What You’ll Bring Current Registered Nurse (RN) license in Massachusetts in good standing Associate’s degree in Nursing required; Bachelor’s degree (BSN) preferred Minimum of 3 years of nursing expe
