Who We Are Imagine Pediatrics is a tech enabled, pediatrician led medical group reimagining care for children with special health care needs. We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. We do not replace existing care teams; we enhance them, providing an extra layer of support with compassion, creativity, and an unwavering commitment to children with medical complexity. While this role is primarily remote, candidate must be living in the tri-state area (NY, NJ, CT, and/or PA). 10 % travel for offsite meetings, training, and educational purposes. 8:00am-5:00pm Monday-Friday with 2 days determined with supervisor being 10:00am-7:00pm. What You’ll Do As a Resource Specialist Care Manager, you will work with the families of medically complex children providing emotional support, care coordination, and resourcing support in navigating the healthcare system. You leverage an integrated technology platform and are complimented by an entire interdisciplinary team including physicians, RNs, care team assistants, mental health therapists, community health navigators, pharmacists, dietitians, and more. In this position, you will: Conduct thorough assessments, exploring the current medical, behavioral, and complex social needs. Identify barriers to care, such as food insecurity, unstable housing, transportation challenges, and insurance limitations. Collaborate with families and multidisciplinary teams to establish individualized care goals. Develop and maintain an integrated care plan addressing medical, behavioral, and social needs. Ensure timely follow-up on referrals, evaluations, and treatment recommendations. Offer education to families about navigating local resources and agencies.
