What They're Looking For.
Must Have
Active unrestricted Registered Dietitian certification issued by the Commission on Dietetic Registration, Minimum of relevant state dietitian licensure or willingness to obtain and maintain additional state dietitian licensures as needed, 2+ years’ experience in the delivery of care to patients with kidney disease or other chronic diseases, Knowledge of healthcare regulations (including HIPAA), Commitment to patient confidentiality, ethics, and professional standards

What You'll Do.
Assess nutritional needs
Develop meaningful care plans
Coach patients through sustainable dietary and lifestyle changes
Conduct virtual nutritional assessments for patients with chronic kidney disease (CKD) and other co-morbid conditions
evidence-based nutrition care plans
Calculate nutrition requirements tailored to the needs and preferences of patients at various CKD stages
Educate patients and their caregivers on kidney-related dietary and lifestyle adjustments
Provide patients with resources and tools to manage their health conditions
Maintain thorough and timely documentation of all patient interactions
Practice within the registered dietitian scope of practice
Make necessary interdisciplinary referrals

Team & Collaboration
Collaborate with interdisciplinary team members, including nurses, social workers, care coordinators, and others, to deliver comprehensive virtual care services to patients; Collaborate with interdisciplinary teams
Communication Scope
communication skills, with the ability to translate complex nutrition concepts into clear, practical, and empathetic guidance for patients and caregivers
