What They're Looking For.
Must Have
Certified Medical Assistant required, Registered Medical Assistant required, Certified EMT required, 2+ years experience, Adherence to patient confidentiality, Reliable transportation, Valid state driver's license

What You'll Do.
Manage targeted population of patients
Assist patient in achieving better health outcomes
Address patient questions and concerns
Connect patients with medical practice
Partner in implementing high-impact care plans
Escalate medical issues
Navigate patients to right resources
Assist with referrals to specialty care
Assist with referrals to CBOs
Document patients' medical history
Document medical issues in EHR
Understand referral process
Understand prior authorization process
Build trusting relationship with medical practices
Build trusting relationship within Guidehealth
Meet assigned Key Performance Indicators
Produce positive patient health outcome
Decrease medical spend
Utilize effective communication modalities
Engage high-risk patients
Schedule high-risk patients
Escalate medical issues to RN
Escalate medical issues to PCP
Escalate medical issues to leadership
Follow security policies
Protect Guidehealth Intellectual Properties
Perform compensated travel
Conduct retinal eye exams
Document patient interviews
Balance competing priorities
Track progress of multiple tasks
Ensure tasks are completed
Work independently as part of care team
Adhere to patient confidentiality
Perform other duties as assigned
