Must Have
Active, nationally recognized Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) certification., MUST live in either MST or PST to be considered., Bilingual fluency in English and Spanish., Current BLS certification., Direct, hands-on experience working with CCM and/or RPM programs., Ability to work independently in a secure, HIPAA-compliant home office., Excellent verbal and written communication skills with the ability to explain complex information simply.
Nice to Have
In-depth knowledge of CMS regulations and documentation requirements for chronic care., Previous experience in care coordination or transitional care., Familiarity with clinical quality measures and reporting.

### Chronic Care Staffing is Hiring a Remote CCM/RPM Enrollment Coordinator! **Location:** Remote in either MST or PST **Pay Range:** $17.00 – $20.00 per hour **Employment Type:** Full-Time ### Overview Chronic Care Staffing is looking for a dedicated Enrollment Coordinator to serve as the bridge between patients and our chronic care programs. In this role, you will be the first point of contact for patients, helping them understand how our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services can improve their health outcomes. This is a vital position that directly impacts patient wellness while supporting our clinical teams. If you are a certified clinical professional who values clear communication and wants to make a measurable difference in patient care from a remote setting, this role is a great fit for you. ### What Success Looks Like You will succeed in this role by mastering the enrollment process and building trust with the patients you support.
