In this role, you serve as a vital link between patients managing long-term health conditions and their clinical care teams. You will provide essential support by conducting monthly outreach, helping patients follow their care plans, and monitoring health data remotely. This is an ideal role for a clinician who wants to make a measurable impact on patient outcomes while enjoying the flexibility of a remote work environment. ### What Success Looks Like Your primary goal is to ensure patients feel supported, informed, and engaged in their treatment plans.
