What They're Looking For.
Must Have
Current registration with College of Nurses of Ontario, Current BCLS certification, Able to practice to full scope, Recent acute care experience, Patient assessment skills, Experience processing physician orders, Sensitivity to caring for acute geriatric client, Ability to adapt quickly, Ability to remain flexible in change, Ability to collaborate within small teams, Able to meet physical demands, Demonstrates being patient and consumer, Communication and interpersonal skills, Demonstrates fiscal responsibility, Access to a vehicle, Valid Ontario drivers license, Physically able to perform duties, Satisfactory employment and attendance record

What You'll Do.
Support home peritoneal dialysis patients
therapeutic relationships
Attain or maintain optimal functioning
Treat health conditions
Perform dialysis independently
Perform associated interventions independently
Provide ongoing follow up care
Provide ongoing support
Practice within scope of Nephrology nursing
Practice within RPN scope
Practice within nursing philosophy
Practice within renal program standards
Provide assessment for home dialysis
Provide education for home dialysis
Provide decision support for home dialysis
Coordinate patient appointments
Facilitate patient appointments
Assess dialysis patients
Manage pre/post dialysis machine
Prepare dialysis circuit
Program dialysis machine
Initiate dialysis treatment
Discontinue dialysis treatment
Monitor patient during dialysis
Monitor machine during dialysis
Provide intervention as necessary
Administer medications as ordered
Engage interdisciplinary team
Act as liaison between care members
Provide framework for transition
Provide smooth transition to home therapies
