Assess patients with diversions
Plan care for patients with diversions
Make nursing diagnoses for patients with diversions
Implement care for patients with diversions
Evaluate care for patients with diversions
Provide continuity of care for ostomy and skin
Improve competence of nursing functions
Serve as a resource person for staff
Place orders for ostomy
Maintain appropriate products for RRMC
Distribute supplies as needed
Meet with manufacturing representatives
Assess and trial new products
Make recommendations to Value Analysis Team
Coordinate Outpatient Ostomy Clinic operation
Maintain safe patient care environment
Comply with RHS policies/procedures/regulatory requirements

Maintain patient and job activity stats daily
Submit stats to Manager/Director monthly
Coordinate with Ostomy Wound Education Program

Chair Skin Integrity Team for RRMC
Teach ostomy patient self-care
Teach significant other ostomy self-care

Full Job Description
Newport News, Virginia _**Labor Pool-Days Available**_ Works under the supervision of the Nurse Manager to assess, plan, make nursing diagnosis, implement, and evaluate care for patients with fecal or urinary diversion, patients with wound and skin problems and pressure ulcers. Provides continuity of care of the ostomy and skin care patient by improving the competence, performance, and understanding of the nursing staff; functions as a consultant for patients with ostomies, complex wounds, fistulas, pressure ulcers, and skin problems, and serves as a resource person for the nursing, allied, and medical staffs, patient, family and community. In addition, the individual must demonstrate knowledge and skills necessary to provide care appropriate to the age of the patient served in their care. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs and to provide the care needed as described in the departmental policy and procedure guide. Assumes responsibility for attending seminars / in-service programs to promote growth and development. May need to be on call at times for departmental needs. **What you will do** * May be required to place orders for ostomy, wound and selected skin care supplies. Maintains appropriate products for RRMC, distributes as needed. Meets with representatives from wound and ostomy manufacturing companies to assess and trial new products and technology; makes recommendations to the Value Analysis Team. * Coordinates the operation of the Outpatient Ostomy Clinic. * Maintains a safe patient care environment by complying with RHS policies/procedures/regulatory requirements (DNV/OSHA/State) that support National Safety Patient Goals and Quality Initiatives.
