Registered Nurse (RN) - Board of Nursing An active license in the state of practice

Certified Hemodialysis Nurse (CHN) - Board of Nephrology Examiners Nursing and Technology (BONENT), Certified Peritoneal Dialysis Nurse (CPDN) - Board of Nephrology Examiners Nursing and Technology (BONENT), Certified Dialysis Nurse (CDN) - Nephrology Nursing Certification Commission (NNCC), Specialty Certification - The American Society of Histocompatibility & Immunogenetics (ASHI), Dialysis experience or critical care experience, 1-3 years nursing experience if functioning as a home dialysis nurse and an additional 3 months experience in each of the specific modalities (hemodialysis or peritoneal dialysis) for which the nurse will provide self-care training
What You'll Do.
Provides care and treatment to patients on maintenance dialysis
Performs and maintains knowledge and proficiency in technical skills for dialysis treatments
Provides oversight and delegation to department staff
Understands and ensures compliance related to water room equipment operation and function
Documents in the patient record according to departmental policy/procedure
Promotes and maintains documentation and an environment that is in compliance with state
and regulatory agencies
including CMS/ESRD-Renal Network
and Quality Assurance and Process Improvement efforts
Coordinates care of patients in collaboration with Medical Director
other nursing staff and all ancillary departments
Acts as a patient/family advocate
Collaborates with the patient
and health care providers in providing patient care
Identifies patient/family learning needs
and evaluates an educational plan for patients
Provides patient education related to
the dialysis process and procedures related to that process
including care and management of dialysis accesses

Be part of a multidisciplinary team; Coordinates care of patients in collaboration with Medical Director, Nephrologist, patients, families, other nursing staff and all ancillary departments; Collaborates with the patient, family, and health care providers in providing patient care; Work effectively in a team environment, coordinating work flow with other team members and ensuring a productive and efficient environment
