What They're Looking For.

Must Have

Current appropriate state licensure, Current or successful completion of CPR BLS Certification, Graduate of an accredited School of Nursing, 12 months experience as a RN, 3 months experience as a RN in home peritoneal dialysis and / or hemodialysis

Nice to Have

1-year experience as a Registered Nurse, Home dialysis therapy experience

What You'll Do.

Train dialysis patients on peritoneal and home hemodialysis

Conduct initial home visits

Function as onsite Patient Educator

Function as Case Manager

Assess and manage patients’ response to home dialysis

Communicate patient related issues to physician

Participate in decision-making

Participate in teaching

Participate in leadership functions

Participate in quality improvement activities

systematic collection and analysis of dialysis

Document patient data in medical record

Make adjustments or modifications to treatment plan

Notify Supervisor or physician

Assess and document patient/family’s basic learning needs

Provide initial and ongoing education to patients and

Direct and provide all aspects of dialysis therapy

Administer medications as prescribed

Document medical justification and effectiveness of medications

Initiate emergency response measures

Assist with emergency response measures

Serve as a resource for health care team

Participate in staff training

Participate in orientation of new staff

Ensure correct laboratory collection procedures

Ensure correct laboratory processing procedures

Ensure correct laboratory shipping procedures

Reschedule missed laboratory collections

Reschedule insufficient laboratory collections

Identify expected outcomes

Document nursing assessment

Update nursing assessment

Document plan of care

Ensure patient awareness related to transplant options

Ensure patient awareness related to treatment modality options

Assist in identification of Home Dialysis candidates

Assist in evaluation of Home Dialysis candidates

Assist in selection of Home Dialysis candidates

Assist in education of Home Dialysis candidates

Assist in education of Home Partners

Perform assessment of Home Dialysis candidate’s home environment

Identify barriers of Home Dialysis candidate’s home environment
