Registered Nurse (RN) - Board of Nursing, Active license in the state of practice
Nice to Have
1-3 years of Clinic Nursing experience, 4-6 years Case Management and /or homecare experience
What You'll Do.
Support patient care navigation
Manage clinic patient care
Develop care management processes
Support primary clinical teams
Identify high acuity patients
Ensure care coordination
Educate patients on self-management
Manage patient care continuum
Collaborate with physicians and staff
Identify patients for care management
Provide leadership for patient care teams
Plan and provide follow-up care
Communicate plan of care
Provide follow-up contact
Review status reports
Communicate statistical and financial impact
Serve as resource for education
Collaborate with payer Case Managers
Participate in team meetings
Anticipate patient needs
Complete pre-visit planning
Empower patients and families
Assess barriers to treatment goals
Determine and complete referrals
Oversee patient self-management resources

Work with all clinical teams; Collaborate with physicians, providers, and practice staff; Work with social services and nursing; Communicate plan of care to families, medical providers, hospital staff, community support, other agencies; Participate in regular team meetings; Work effectively in a team environment; Coordinate work flow with other team members
