Must Have
BLS Basic Life Support, RN License, 1 year of experience in clinical setting
Nice to Have
Accredited Case Manager Certification (ACM), Certified Case Manager (CCM), ANCC Nursing Case Management board certification, 3 years of experience in an acute care clinical setting, Ambulatory or post-acute, care coordination experience
What You'll Do.
Provide coordination of care for patients

Monitor and revise plan
Complete all necessary documentation
Utilize handovers at points of care change
Reflect plan of care for post hospital needs
Incorporate patient goals and preferences
Follow standardized practices for Advance Care Planning
Support denial prevention related to medical necessity
Address/remove barriers to progression of care
Participate in Interdisciplinary Discharge Rounds
proactive care for patients
Assist in removing barriers to timely testing
Ensure resources are utilized appropriately
Offer alternatives to acute care
