Full Job Description
- Role: Registered Nurse (RN) - Case Manager - Comp: $85,000-$105,000 approximate yearly pay - $64.10 per point - Mileage in addition to productivity comp - Schedule: Full-time Monday–Friday + some weekends (as needed) - Location: Field-based WE'RE FIXING HOME HEALTHCARE, AND WE'RE BUILDING OUR PRESENCE IN DETROIT Adaptive is building an AI-native home healthcare platform that enables clinicians to focus on delivering best-in-class care for their patients by leveraging software and innovative support solutions. We’re building our team in Detroit with plans to quickly develop an exceptional reputation of delivering excellent patient care. THE MISSION OF THIS ROLE Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on. OBJECTIVES 1. Carry a full, high-quality caseload: ramp to ~30 productivity points/week at target visit quality, with OASIS accuracy that holds up to review. 2. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality. 3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent. 4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows. WHAT YOU'LL OWN - Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge). - Direct, high-acuity patient care, with a focus on wound care - Building and managing the plan of care with physicians; supervising LPNs and HHAs on your cases. - Being the face of Adaptive to patients, families, and the referral sources who feed your pod.
