What They're Looking For.
Must Have
Current Texas RN license, 3+ years clinical nursing experience, Comfort with wound care and infusions, CPR required (BLS), Current Texas drivers license, Comfort with field-based work, Time management and organization, Clear, professional communication
Nice to Have
Previous home health experience, Familiarity with Homecare Homebase (HCHB), Experience coordinating care for medically complex patients, ER or equivalent nursing experience
What You'll Do.
Own patient assessments
Own skilled visit documentation
Deliver high-acuity home health care
Focus on infusion therapy
Maintain plan of care
Communicate with physicians
Communicate with care team members
Communicate with patients
Communicate with families
Serve as patient advocate
Ensure continuity of care
Drive case management responsibilities
Keep patients progressing
Communicate changes in condition
Administer medications
Administer treatments
Provide patient teaching
Provide caregiver teaching
Coordinate with internal teams
Deliver care reliably
Provide supervision to LVNs
Provide guidance to LVNs
Provide supervision to HHAs
Provide guidance to HHAs
Participate in case conferences
