What They're Looking For.
Must Have
Graduate of an accredited School of Nursing, Current appropriate state licensure, Current or successful completion of CPR BLS Certification, Must meet the practice requirements in the state in which he or she is employed, Entry level for RNs with less than 2 years of Nephrology Nursing experience as a Registered Nurse
Nice to Have
Minimum of 1-year experience as a Registered Nurse, Home dialysis therapy experience
What You'll Do.
Train dialysis patients on peritoneal and home hemodialysis
Conduct initial home visits
Function as an onsite Patient Educator
Function as a Case Manager
Assess and manage patients' response to home dialysis
Communicate patient related issues to physician
Participate in decision-making
Participate in teaching
Participate in leadership functions
Participate in quality improvement activities
Collect and analyze dialysis data
Document patient medical record
Assess and document patient/family learning needs
Provide education to patients and family
Administer medications as prescribed
Initiate or assist with emergency response measures
Serve as a resource for health care team
Participate in staff training
Orientation of new staff
Ensure correct laboratory collection procedures
Update the nursing assessment and plan of care
Ensure patient awareness related to transplant options
and educate Home Dialysis candidates
Assess barriers of the Home Dialysis candidate’s home environment
Train Home Dialysis patients on equipment operation
Train Home Partners on equipment operation
Complete CAP requirements to advance
