Heavy lifting, Moving of equipment and patients, Assisting with ambulation, Frequent, prolonged periods of standing, Bend over, Move machines and equipment of up to 200 lbs, Lift chemical and water solutions of up to 30 lbs, Exposure to infectious and contagious diseases/materials, Rotates coverage with other licensed home therapies staff, Participation in on-call rotation, Night, weekend, holiday coverage, Travel to training sites, other facilities or patient homes, Provide essential functions in other locations including patient’s home
What They're Looking For.
Must Have
Graduate of an accredited School of Nursing, Current appropriate state licensure, Current or successful completion of CPR BLS Certification, 12 months experience as a RN, 3 months experience as a RN in home peritoneal dialysis and / or hemodialysis
Nice to Have
1-year experience as a Registered Nurse, Home dialysis therapy experience
What You'll Do.
Train dialysis patients on peritoneal and home hemodialysis
Conduct initial home visits
Assess and manage patients' response to home dialysis
Communicate patient related issues to physician
Participate in decision-making
and quality improvement activities
Collect and analyze dialysis data
Document patient medical record
Assess and document patient/family learning needs
Provide education to patients and family
Provide safe and effective delivery of dialysis therapy
Administer medications as prescribed
Initiate or assist with emergency response measures
Serve as a resource for health care team
Participate in staff training and orientation
Ensure correct laboratory collection procedures
Reschedule missed laboratory collections
Update the nursing assessment and plan of care
Ensure patient awareness related to transplant options
Assist in education of Home Dialysis candidates
Perform assessment of Home Dialysis candidate's home environment
Train Home Dialysis patients on equipment and supplies
Complete CAP requirements to advance
Perform all other duties as assigned

Collaborate with another Registered Nurse; Collaborate with the Interdisciplinary Team; Collaborate with patient, home care partner, direct and ancillary patient care staff
