What They're Looking For.
Must Have
Graduate of an accredited School of Nursing, Current appropriate state licensure, Current or successful completion of CPR BLS Certification, Minimum of 12 months experience as a RN, Successful completion of 3 months experience as a RN in home peritoneal dialysis and / or hemodialysis
Nice to Have
Minimum of 1-year experience as a Registered Nurse, Home dialysis therapy experience
What You'll Do.
Provide and coordinate clinically competent care
Assess and manage patients' response to home dialysis
Communicate patient related issues to physician
Participate in decision-making
Participate in quality improvement activities
systematic collection and analysis of dialysis
Document in the patient medical record
Make adjustments or modifications to treatment plan
Notify Supervisor or physician as needed
collaborate and document patient/family’s basic learning needs
Provide initial and ongoing education to patients and
Direct and provide all aspects of dialysis therapy
Administer medications as prescribed
Document appropriate medical justification and effectiveness
Initiate or assist with emergency response measures
Serve as a resource for health care team
Participate in staff training and orientation
Ensure correct laboratory collection
processing and shipping procedures
Reschedule missed or insufficient laboratory collections
Identify expected outcomes
Document and update the nursing assessment and plan
Ensure patient awareness related to transplant and treatment
Assist in the identification
selection and education
Perform assessment and identify barriers of the Home
Train Home Dialysis patients on equipment operation and
