We’re hiring a Registered Nurse (RN) – Admissions – a compassionate and skilled clinician who performs comprehensive evaluations, initiates care plans and ensures a smooth transition for patients entering hospice. This role is essential to delivering dignified, patient-centered care and supporting families during critical moments. What You’ll Be Doing Perform comprehensive nursing evaluations to admit patients into hospice Assess for signs and symptoms of discomfort, including pain, and take prompt, appropriate action Initiate care plans and provide appropriate nursing follow-up after hours Instruct caregivers on managing patient needs and submit documentation within 24 hours of visit Coordinate all services provided to patients and families across settings to ensure quality and appropriateness of care Follow approved plans of care, modify as needed, and communicate with physicians when appropriate Qualifications Registered Nurse with a valid state license Minimum of 1 year nursing experience required; hospice experience preferred Ability to lift up to 50 pounds Must clear criminal background check, physical, and drug screening Valid driver’s license, good driving record, and reliable transportation required W
