This role blends clinical care, patient education, and case management. **_Flexible_ Monday-Friday 8am-5pm type-of schedule**. **_No Weekends, No Holidays!_** Will be covering census in one facility; but **must be flexible to support in other surrounding home facility centers as needed including but not limited to:_Stamford, Norwalk, Westchester, Yonkers East, and Tarrytown_** _._ **Excellent Paid Training Provided!** Dialysis Experience is NOT required. Nephrology nursing provides a fresh lens and set of skills to those newly entering the field. **Key Responsibilities:** * Train patients and caregivers on PD procedures and self-care * Manage a caseload of PD patients, including assessments and care planning * Monitor treatment progress, conduct home visits, and provide on-call support * Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes **Schedule:** * Monday–Friday, 8-hour shifts * Weekends and holidays off * Flexible scheduling to manage your own caseload **Requirements:** * RN license in the state of practice * Willing to obtain both CT and NY State license (we reimburse) * **Minimum of 12 months of RN experience** * Valid driver’s license, insurance, and reliable transportation * ADN required; BSN preferred * Basic computer skills (MS Word, Outlook) **Preferred Experience:** * Dialysis (PD, HHD, chronic, or acute) * Patient education or teaching background * Case management or telehealth experience * CNN or CDN certification **What We Offer:** * Medical, dental, vision, and 401(k) match * Paid time off and PTO cash-out * Mental health and family support (EAP, Headspace, parental leave, etc.) * Paid training and ongoing professional development Empower patients to take control
