Associate degree in nursing from an accredited college or university (or international equivalent) preferred, One (1) year of home care and/or Hospice experience preferred
What You'll Do.
Provide professional nursing care in accordance with the State Nurse Practice Act and hospital policies
Conduct initial and ongoing patient assessments
including vital signs
and review of medical history
and evaluate individualized care plans based on patient needs
Administer medications and treatments as prescribed by physicians
Monitor and document patient status
including changes in condition
and response to treatments
Provide patient and family education regarding disease management
and self-care techniques
Assist patients with activities of daily living to promote comfort and independence
Ensure patient safety and maintain a therapeutic and supportive environment
Maintain accurate and timely documentation in the electronic health record

Team & Collaboration
Collaborate closely with physicians, therapists, and interdisciplinary team members to coordinate care; Participate in daily rounds, interdisciplinary team meetings, and patient care conferences
