What They're Looking For.
Must Have
Associate's degree required, New Jersey State Nursing License required, BLS certification required, 0 – 1 years experience
Nice to Have
Bachelor’s degree preferred, 2-4 years experience preferred, Experience with Standard Office Equipment preferred, Experience with Standard Office Technology in a Window based environment preferred, Experience with patient care equipment preferred, Experience with EHR, Practice Management System and Departmental/Ancillary Systems preferred
What You'll Do.
Assist physician and staff with patient care coordination
Provide individualized team-based care
Deliver direct patient care within scope of licensure
Collect lab specimens
Perform Point of Care testing
Prioritize patient care
Prepare for patient visit
Manage Nurse Visit schedule
Perform patient intake
Check-in/check-out patients
Communicate problems and concerns
Identify patients prior to care
Manage Clinical Nurse inbox
Administer injections
Perform patient outreach
Coordinate and facilitate daily huddles
Demonstrate telephone triage protocols
Schedule appointments
Identify and schedule annual visits
Monitor daily hospital discharge list
Adhere to universal safety precautions
Review and update medication list
Initiate prior authorization requests
Enter orders in PMS and EHR
Provide referrals and consult orders
Check and maintain logs for emergency equipment
Facilitate transition of care
Maintain proficiency with department equipment
Participate in pilot programs
Assist in ordering and maintaining supplies
