Must Have
Graduate of an accredited school of nursing, Current licensure in State, CPR certification, Licensed driver with automobile
Nice to Have
1-2 years acute care experience, 1-2 years home care experience, Bachelor's degree, 1 year hospice care experience
What You'll Do.
Complete patient and family assessment
Provide physical assessment and history
Provide professional nursing care
Assess and evaluate patient status
Write and initiate plan of care
Re-evaluate patient and family needs
Revise plan of care as necessary
Initiate plan of care
Make necessary revisions to plan of care
Use health assessment data
Determine nursing diagnosis
Establish goals based on diagnosis
Incorporate palliative nursing actions
Include patient and family in planning
Initiate preventive and rehabilitative nursing procedures
Administer medications and treatments
Counsel patient and family
Provide health care instructions
Assist patient with activities of daily living
Facilitate patient's self-sufficiency
Facilitate patient's comfort care
Coordinate patient care
Complete clinical notes
Maintain clinical notes
Submit clinical notes
Record pain/symptom management changes
Record pain/symptom management outcomes
Communicate with physician
Report patient changes
Obtain physician orders
Receive physician orders
Communicate with community health persons
Teach patient and family self-care
Provide medication instructions
Provide diet instructions
Provide other instructions
Recognize health counseling opportunities
Utilize health counseling opportunities
Work in concert with interdisciplinary group
